MOC Points Application & resources

Use the forms & resources below to have your continuing medical education (CME) activity certified for Maintenance of Certification (MOC) Points with the ABA, ABIMABOABOHNS, ABP, and/or ABPath. Your activity must first be approved for AMA PRA Category 1 CreditsTM by the University of Tennessee College of Medicine (UTCOM) via our CME Credit Application in order to receive MOC Points from the UTCOM. 

 

MOC Part 2 Points Application - Use this form to apply to have your educational activity certified for MOC Part 2 Points from the ABAABIM, ABOABOHNSABP, and/or ABPath. Get tentative approval from your Office of CME before completing this application. 

 

MOC Points Application Resources

Required Attachment #1) Learners Assessment - We don't have a standard template for it, but you need to administer an assessment of the learner's knowledge and/or skill that includes a justifiable passing standard according to the 2 peers you have review this activity, and a simple Post-Test is the easiest way to satisfy this requirement.  We will be glad to help you craft your Learner Assessment, and to show you past examples from other CME activities. 

 

Required Attachment #2) Learner Feedback - We don't have a standard template for it, but you need to provide feedback to your learners that includes a) the results of their assessment (Attachment #1 above), b) the rationale for correct answers or attainment of skills, and c) citations where appropriate. We will be glad to help you craft the feedback you will provide to your learners, and to show you past examples from other CME activities. 

 

Required Attachment #3) MOC Points Credit Request form template - Our MOC Points Credit Request form template includes the basic information & permission you need to gather from attendees in order for us to confirm their participation and communicate their MOC Points to their specialty medical board. 

 

 

MOC Points Closeout Resources

Required Attachment #4) MOC Points Participation Report - The ACCME requires a specially-formatted attendance/participation report to be used in order to submit MOC Points completions. Download & save our MOC Points Participation Report template and use it to submit participation info to us for each learner at your MOC Points activity. 

 

Once your UTCOM-certified CME activity is over, visit the CME Activity Closeout webpage for guidance in submitting the closeout materials needed to issue the CME credit certificates/transcripts.
